This is a form for a project organised by Notes from Below. By submitting information through this form, you give your consent for that information to be used by Notes from Below. Survey responses are confidential and no identifiable information will be made publicly available.

Your responses to the survey will be collected and stored electronically. By filling out this form, you consent to transmitting your data and having it saved in a database on the server of the platform hosting this data. You consent to this data being downloaded and processed by members of the Notes from Below team.

The legal basis for the collection of this data with respect to users in the EU is Art. 6 para. 1 lit. a GDPR (data subject’s consent). Your personal data will not be shared with any third parties outside of the Notes from Below team, and will be deleted after five years.

Additionally, you have the right to withdraw your information, without giving a reason, at any time by contacting Notes from Below on [email protected]. Please note that we will not be able to remove your aggregated response from any analysis we have already undertaken on the data you have submitted, but no individual will be able to be identified from aggregated data.
